Incident Reporting
Students in the Midwifery Education Program (MEP) must report any clinical incidents they are involved in during their placements and also any incidents which result in their own personal injury. There are two different processes for these incidents.
Clinical Incidents
Students involved in clinical incidents during clinical placements should seek support from their preceptor and/or tutor to fill out the PDF fileStudent Incident Report Form. The form should be submitted without delay to the MEP program manager.
Personal Injury Reporting
Workplace Safety and Insurance Board (WSIB) reporting is required if the injury/incident may result or has resulted in any or all of the following:
- Health care by a medical practitioner for which there is a fee for service (i.e. physician, chiropractor, dentist or hospital)
- Loss of time from work, placement and/or university beyond the date of the incident
- Exposure to and/or contraction of a communicable disease (i.e. TB exposure, needlestick)
In the event of a practice injury/incident requiring first aid treatment only, you do not need to make a WSIB claim.
Please follow placement agency/facility protocol for reporting and obtaining personal medical attention. If you seek additional medical attention or are uncertain about the potential severity of the incident, please obtain, complete and submit the WSIB documentation, as instructed below.
First aid is defined by the WSIB as a “one-time treatment or care and any follow-up visit(s) for observation purposes only. First aid includes, but is not limited to: cleaning minor cuts, scrapes, or scratches, treating a minor burn, applying bandages and/or dressings, applying a cold compress, cold pack, or ice bag, applying a splint, and changing a bandage or a dressing after a follow-up observation visit.”
Select the topic, below, that is most appropriate for your injury or incident and follow the instructions provided. Your faculty advisor or clinical instructor will assist you in completing the paperwork.
If the practice incident is an accident or injury that necessitates WSIB reporting, please follow placement agency/facility protocol for reporting and obtaining personal medical attention. If additional medical attention is desired, seek attention from your family practitioner or the Toronto Metropolitan University Medical Centre.
Notify the practice instructor or faculty advisor as soon as possible on the day of the incident.
Obtain, complete and submit the following WSIB documentation with your faculty advisor or clinical instructor.

To Submit First-Aid Paperwork
- Photocopy any documentation reflective of the first aid incident (i.e. occupational health department report) and retain the original copy for your personal records.
- Submit photocopied documents to the MEP in a sealed envelope.
