Request for Consideration for Medical or Compassionate Reasons
Requests for consideration for medical or compassionate reasons need to be submitted via the Senate portal. Do not send these directly to faculty or staff. Complete the following steps for health-related academic consideration requests:
- Have your primary healthcare provider complete the PDF fileStudent Health Certificate
- Complete the PDF fileAcademic Consideration Submission Form
- Submit your request through the Senate's Office online submission portal
Confirmation of Enrollment
There is a $20 fee for Confirmation of Enrollment letters. Please provide cash or a cheque/money order made out to the Midwifery Education Program with your request. If you would like to have your letter mailed to you instead of picking it up at the Midwifery Education Program office, please provide the mailing address. Please allow five business days for processing.
