General forms for the Yeates School of Graduate and Postdoctoral Studies can be found in the main YSGPS forms page. Below are some of the more commonly used forms for students in the Molecular Science graduate program.

Form Description
Student/Supervisor Checklist Due by the end of the first month of registration this form communicates the responsibilities of students and supervisors.
 (PDF file) Plan of Study Due by the end of the first month of registration this form is used to establish the plan and timeline for courses and/or non-course requirements that a student is expected to follow in order to complete their degree requirements.
Stipend Form This form is to be completed by the student and supervisor and submitted to the program office to initiate stipend payments to the student through the HR payroll system.
 (PDF file) Supervisory Committee Nomination and Approval Completed in consultation with the supervisor and approved by the Graduate Program director, this form is due by the end of the first term of registration in the program.  
 (PDF file) Progress Report This report is used to maintain a record of student progress; to assign a term performance designation to the thesis; and in the awarding of teaching assistantships and research fellowships. Students must complete this report every term they are registered in their program. Reports are due at the start of the following term.
 (PDF file) Thesis Proposal and Supervisory Committee Report Completed by the supervisory committee at the first supervisory committee meeting, this report is used to approve the MSc thesis proposal. It is due by the end of the second term of study.  
 (PDF file) Supervisory Committee Report Completed by the supervisory committee at the second and third committee meetings which are held in the student's fourth and fifth terms of study.
 (PDF file) Examination Recommendation & Scheduling This form is completed in consultation with the supervisor when a student is ready to schedule their defence.
 (PDF file) Supervisory Report Form for PhD Transfer Applicants Completed by the supervisory committee when recommending a student for transfer to the PhD program, typically in the fourth term of study. This form is to be included with the transfer application package. Consult the PhD transfer guidelines for more information.
 (PDF file) Molecular Science Program Transfer Application Form This form is to be submitted with the PhD Transfer application package. Please consult the PhD Transfer guidelines for more information.
 (PDF file) Verification of Thesis/Dissertation Revisions Form This form is completed by the supervisor and submitted to the program office after the student has completed revisions to their thesis as advised by the defence committee.
NLC form This form is to be completed by the student when they submit their final revised thesis to the program office after their defence.
Program Level Appeal This form is to accompany documents supporting your academic appeal at the program level and is to be submitted to the program office by the deadline as indicated by YSGPS significant dates.
 (PDF file) Directed Studies Course Approval Form Students and supervisors complete this form and submit to the program office for approval by the GPD by the end of the first week of registration for the term they wish to be enrolled in MS8109/MS9210.
Molecular Science Travel Grant Please submit your receipts to the FOS RESPECT portal. Program reimbursements will be processed together with reimbursements from your supervisor if applicable. Please speak with your supervisor for details about their contribution to this travel expense.

Directed Studies Course Approval Form (external link)  Students and supervisors complete this form and submit to the program office for approval by the GPD by the end of the first week of registration for the term they wish to be enrolled in MS8109/MS9210.
Supervisory Committee Report (external link) 
Completed by the supervisory committee. PhD students are expected to meet with their supervisory committee once every year that they are enrolled in the program, typically during the third, sixth, ninth and twelfth term of study.  
Candidacy Examination Committee Approval Form (external link)  Due no later than the end of the fifth term of study this form initiates the candidacy examination process. Please consult the candidacy exam guidelines for more information.  
Report of the PhD Candidacy Examining Committee (external link)  Completed by the examining committee after the oral candidacy exam, this form is submitted by the committee chair to the program office.
External Examiner Nomination In consultation with the supervisor, this form is to be submitted to the graduate program office with a current CV for the nominee who is to be part of the dissertation examining committee for a PhD defence. This form can be submitted together with the Dissertation Examining Committee Nomination and Approval form to the program office for approval by the GPD and the Dean, YSGPS.
Dissertation Examining Committee Nomination and Approval In consultation with the supervisor, this form is to be submitted to the graduate program office to nominate a dissertation examining committee for a PhD defence. This form can be submitted together with the External Examiner Nomination Form to the program office for approval by the GPD and the Dean, YSGPS.
Dissertation Scheduling Request (external link)  Due at least 3 weeks before your requested defence date, submit this form to the program office upon notice of approval from the Dean of YSGPS of your External Examiner and Examination Committee nominations.
