If you need to submit additional Ontario Student Assistance Program (OSAP) forms or appeal your OSAP funding, TMU can help you explore your options.

Forms and Documents

If you're looking to apply for the Ontario Student Assistance Program (OSAP), you can do so online via the OSAP portal (external link) All OSAP documents must be uploaded directly to your OSAP portal. To protect your safety, our Student Financial Assistance office at Toronto Metropolitan University (TMU) doesn't accept documents by mail, fax or in person.

If you need to submit OSAP forms to complete your application, log in to your OSAP application. On your Required Documents page, click on the "View Documents & Print/Upload" button. From there, you'll be directed to download, scan and upload required documents for your OSAP file.

You can also access all the OSAP forms you might need on the Government of Ontario linked below.

On this page is a consolidated list of commonly-accessed OSAP forms that we've put together so you can find everything in one place.

This list also includes some TMU-specific forms you might need for your application that aren't available on the OSAP website.

  

Form Deadlines and Important Dates

Be mindful of submitting your OSAP documents within the important dates and deadlines. Once submitted, we guarantee a 3 to 6 week turnaround for review. You can keep track of your documents' progress through our OSAP Processing Dates page. As your documents get reviewed, updates will be made available via your OSAP Message Centre.

  

OSAP Forms

Form When to Use This Form

To be considered for the Ontario Bursary for Students with Disabilities (BSWD) and/or the Canada Student Grant for Services and Equipment for Persons with Permanent Disabilities (CSG-PDSE).

If you're a dependent student, you must provide parental information on your application. An Extenuating Parental Circumstances form may be considered if parental information is unavailable because of the following:

  • Estrangement
  • Situations like political unrest or civil war in the country where they live
  • They are in a refugee camp or their whereabouts are unknown because of the political climate in their country

 (PDF file) OSAP: Studies at Another Institution If you're taking the majority of your courses at TMU and additional courses at another institution through a Letter of Permission.
 (PDF file) 2026-2027 Acknowledgement of Academic Probation (external link)  If you're on academic probation or are returning to school after a period of academic probation.
Form When to Use This Form

Part-Time OSAP Academic Progress Letter

If you were placed on OSAP academic progress through full-time OSAP and you're now applying for part-time OSAP as a result. 

Part-Time OSAP Zero Income Attestation

If you've applied for part-time OSAP and indicated that:

  • You didn't file Canadian taxes for the previous taxation year
  • You didn't earn any income in Canada during the previous taxation year

If you need to maintain interest-free status on your previous part-time OSAP student loans. You must be currently registered in a part-time course load in order to maintain interest-free status through Schedule 2.

Get Reminders and Messages

Check your OSAP Message Centre in your portal at least once a week for important updates.

Appeals and Reviews

If you're not receiving enough OSAP funding, you might be eligible for an OSAP review. You can request this if you have a special situation that isn't recognized in the standard OSAP assessment of financial need. 

Note that submitting an OSAP appeal or review doesn't guarantee additional funding. 

Below is a consolidated list of commonly-accessed OSAP appeals and reviews that we've put together so you can find everything in one place.

Although we can review a number of items, there are a few situations that don't qualify for review, including:

  • If you've received maximum OSAP
  • Your parent(s) arbitrarily refuse to contribute to your education-related costs
  • Your citizenship status
  • Independent status criteria 
  • Deadline dates
  • Funding distribution

Note: OSAP reviews must be submitted 40 days before the end of your study period. For a complete list of deadline dates at TMU, visit Important Dates and Deadlines.

  

Appeal and Review Forms

Form When to Use This Form
If you're a student:
  • Not living with your parent(s) even though they live within 30km of TMU
  • Living with your parent(s) and paying room and board
  • Married, but not living with your spouse during the study period due to location

If you have additional local travel costs during your study period.
Form When to Use This Form
 (PDF file) Exceptional Circumstances Review (external link) 

If you're a full-time OSAP recipient and have:

  • OSAP overpayment
  • OSAP overpayment restriction 
  • OSAP grants that have been, or will be, converted into a loan due to dropping courses or withdrawing from your studies

Note: This is under the authority review of the Ministry of Colleges, Universities, Research Excellence and Security (external link) . Once all required documents are submitted, they'll be forwarded to the Ministry. You'll receive a notification on the decision after 8 to 10 weeks.

Personal Circumstances Review

If you were unable to meet OSAP academic progress due to personal circumstances beyond your control, you may be able to appeal your academic progress standing.

Submit the following documentation based on your circumstances:

  1. A letter explaining why you're requesting this appeal
  2. Medical condition
    1. Documentation provided by a physician that demonstrates your ability to participate in university studies was restricted for a part of, or the entire, study period during which you didn't meet OSAP academic progress
    2. Documentation that shows you're now well enough to continue your studies on a full-time basis (e.g. assignment grades, academic study plan, etc.)
  3. Disability
    1. Relevant medical documentation associated with your disability diagnosis (optional)
    2. A letter outlining: 
      1. If your disability was diagnosed before the start of the study period during which you didn't meet academic progress. If applicable, explain what supports and accommodations you accessed. If you didn't access any, provide the reason for not seeking/receiving those supports 
      2. Outline the supports and strategies you have in place to address your disability-related needs for academic success (e.g. registered with Academic Accommodation Support, asked for advice from your program department about taking a reduced course load, etc.)
    3. Documentation regarding your participation in accessing resources and support for your academic studies (e.g. Academic Accommodation Support, tutoring, etc.)
  4. Family Crisis
    1. A letter outlining your circumstances
    2. A letter from a third party (e.g. a relative, a friend, a physician, etc.) confirming the circumstances

If your parent(s)’ income changed substantially since 2025, you might be able to have your OSAP based on their estimated income for 2026.

Access Fillable PDF Forms

Many forms are in PDF format and can be completed digitally. To download the file:

  1. Right-click on the link and select "Save link as…"
  2. Save the link to your desktop.
  3. Open your Adobe Acrobat Reader. If you don’t have it, you can download it for free through CCS Adobe at TMU.
  4. In Acrobat, go to the File menu, select "Open" and navigate to the file you just saved.
