You may submit this form if you applied to a TMU undergraduate program and you have not received an admission decision (approval, denial, wait list, cancellation). This form is not available for applicants to the Faculty of Law or Graduate Studies.
If you believe that your academic record may not be an accurate reflection of your academic achievement(s) or potential, you are welcome to submit this form. You may use it to inform the Undergraduate Admissions Committee of an experience(s) or circumstance(s) that impacted your previous academic performance such as personal illness, mourning/bereavement, extracurricular and leadership commitments and employment/financial obligations. You may also provide insight into any other barriers you have faced (e.g. social or personal) that have impacted your academic success and your pathway to university.
Please note that this form is not intended for the updating of academic records/history, the submission of additional transcripts, or as an expression of interest in a program(s).
You may attach documents to this form as they relate to your circumstance(s), such as medical documentation of an illness, letters of support for extracurricular commitments, etc. Please do not attach academic transcripts.
Information provided on this form will be kept confidential and will not be provided to any other department at TMU, including the Academic Accommodation Support office.
We encourage you to submit this form by February 1st. Information received after this date will be considered, although subject to space availability in our programs.
We endeavour to consider all requests submitted through this process, however due to the competitive admission and selection process, and the limited space in our programs, only those applicants selected by our Undergraduate Admissions Committees will receive an Offer of Admission. TMU does not issue alternate Offers of Admission to applicants who are not selected for admission. TMU is committed to reviewing each situation on a case by case basis, in keeping with the principles of fairness and equity for all applicants.
