Learner Mistreatment Disclosure

This confidential tool is for undergraduate and postgraduate learners at TMU’s School of Medicine to disclose any behavior they have experienced or witnessed in the learning environment that they perceive or suspect to be mistreatment.

Mistreatment can take many forms, including verbal abuse, discrimination, exclusion, humiliation, or denial of accommodations. This form allows you to confidentially or anonymously share experiences that have affected your learning or well-being. Submitting this form does not trigger a formal complaint unless you request it. Support is available.

If you are a staff member, clinical faculty, peer, friend, or family member who has witnessed mistreatment or is concerned about a SoM learner, you can submit a third-party disclosure using this form. Please indicate in the form that you are submitting on behalf of someone else. The designated advisor in Learner Affairs will review the information and, where possible, reach out directly to the learner to offer support and discuss available options.

Even if you cannot share every detail, your disclosure helps create a safer, more respectful learning environment. While anonymous third-party submissions limit our ability to follow up directly with the learner, they still contribute to institutional awareness and continuous improvement.

Please note: This form is not for emergencies. If you or someone else is at immediate risk of harm, please call 911 or contact campus security or emergency services. Submit this form only after safety has been ensured.

Anonymous Disclosure

  • You can choose to submit a disclosure without providing your name or contact information. If you go this route, no one will follow up with you directly.
  • Because we won’t be able to follow up or ask for more detail, our ability to look into the situation or take specific action may be limited. But even anonymous disclosures help us identify concerns, track trends, and improve the learning environment for everyone.
  • Your submission will go to the designated advisor in Learner Affairs and will be added to a confidential database that helps us monitor and better understand patterns of mistreatment over time.

Confidential Disclosure

  • If you choose to provide your contact information, a designated advisor from Learner Affairs will contact you by email within 72 hours. Their role is to support you, listen to your experience, and help you understand what your options are based on what you’re hoping for.
  • Your information will not be shared without your consent, unless there’s a concern about safety or a legal duty to report. In some cases, depending on what you’d like to happen next, we may ask if you’re open to sharing more details or participating in a separate process (for example, if the issue involves sexual violence or discrimination). We’ll walk you through all of this when we connect with you, and you’re always in control of how you'd like to move forward.
