Report an Incident or Injury

All incidents—regardless of severity—that result in either injury or illness, and/or damage to Toronto Metropolitan University property, must be reported. This allows members of the university to investigate promptly and implement corrective measures to prevent a similar recurrence.

Critical injuries

It is important to know what to do in critical injury situations. Critical injuries are defined under the Occupational Health and Safety Act as injuries that:

  • place life in jeopardy;
  • make someone unconscious;
  • result in a substantial loss of blood;
  • cause the amputation of a leg, arm, foot, or more than one finger or toe;
  • cause burns to a major portion of the body;
  • cause the loss of sight in an eye; and/or
  • cause a fracture of an arm, leg, wrist, hand, ankle, foot, or more than one finger or toe.

If a critical injury has occurred

If a critical injury has occurred, do not disturb the scene. Follow these steps:

  1. Call 911 for emergency medical assistance.

  2. Call TMU Security at 416-979-5040. They will provide assistance and coordinate with local authorities as necessary. They will also be responsible for securing the site.

  3. Call Environmental Health and Safety (EHS) who will be responsible for reporting it to the Ontario Ministry of Labour immediately by phone in accordance with the OHSA.

In the case of fatality, they will also be required to notify the Coroner's Office.

If an employee has been hurt

If and when an employee is involved in an incident or injury that requires medical care or time off work, the employee is covered by the Workplace Safety and Insurance Board (WSIB).

If you are an employee who has been in an incident or injury while on campus, follow these steps:

Step 1: Seek first aid or medical emergency assistance as needed

Call 911 for emergency medical assistance.

You can also call TMU Security at 416-979-5040, 24 hours a day, 7 days a week.

Step 2: Notify your supervisor immediately

If you are able, please contact your supervisor immediately.

Even if the injury does not require medical attention or lost time, your supervisor should be notified.

Step 3: Cooperate with your supervisor to complete the paperwork

Your supervisor will be required to complete the  (google form) Internal Near Miss, Incident and Injury Form (external link)  with your cooperation. This form will be submitted to Environmental Health and Safety on your behalf.

If your injury requires medical care or time off work

If your injury requires medical care or time off work, your supervisor will be required to complete the Workplace Safety and Insurance Board (WSIB)  (PDF file) Form 7: Employer’s Report of Injury/Disease (external link)  and submit to HR within 48 hours.

During your recovery, if you are under the care of a medical practitioner and workplace accommodations are required, please contact Workplace Wellbeing Services.

If you are a supervisor with an employee who has been in an incident or injury while on campus, follow these steps:

Step 1: Go to the scene immediately

As the supervisor, you are obligated to go to the scene as soon as you are notified to assist and control other potential incidents or accidents where necessary.  

Step 2: Call first aid or medical emergency assistance for the injured as needed

Call 911 for emergency medical assistance.

You can also call TMU Security at 416-979-5040, 24 hours a day, 7 days a week.

If there is a critical injury, do not disturb the scene. Follow the steps above.

Step 3: Investigate the incident

Promptly investigate the incident to determine the cause and take appropriate corrective action to prevent a recurrence.

Step 4: Complete the Internal Near Miss, Incident and Injury Form

Complete the  (google form) Internal Near Miss, Incident and Injury Form (external link)  within 24 hours, or as soon as you are aware of the incident.

If the injury requires medical care or time off work

If the injury requires medical care or time off work, you will be required to complete the Workplace Safety and Insurance Board (WSIB)  (PDF file) Form 7: Employer’s Report of Injury/Disease (external link)  within 48 hours.

Make two copies of the completed form: send one to the employee and the other to TMU Human Resources.

During the employee’s recovery, please work with Workplace Wellbeing Services to manage your employee’s safe and early return to work.

If a non-TMU student trainee has been hurt

If a student trainee or learner from an external Ontario educational institution completing an unpaid work placement at TMU has been involved in an incident or injury, the student trainee has workplace insurance coverage paid by the Ministry of Colleges and Universities (MCU).

If you are a non-TMU student trainee who has been in an incident or injury while on campus, follow these steps:

Step 1: Seek first aid or emergency medical assistance as needed

Call 911 for emergency medical assistance.

You can also call TMU Security at 416-979-5040, 24 hours a day, 7 days a week.

Step 2: Notify your TMU supervisor and your institution immediately

Notify your TMU supervisor or others in charge (e.g. professor, instructor, teaching assistant, chair, academic director, event organizer, project manager, etc.) and your own educational institution immediately following the incident or injury. Even if it is an injury which does not require medical attention or lost time, it is important that you notify them.

Step 3: Cooperate with your TMU supervisor and placement coordinator and your institution to complete the necessary paperwork

Cooperate with any requests made by Workplace Safety Insurance Board (WSIB) and complete the necessary paperwork as requested.

If you are a placement supervisor with a student trainee who has been in an incident or injury while on campus, follow these steps:  

Step 1: Go to the scene immediately

As the supervisor, you are obligated to go to the scene as soon as you are notified to assist and control other potential incidents or injuries where necessary.

You must also notify any relevant department managers, deans or chairs involved in the student-trainee’s placement.

Step 2: Call first aid or emergency medical assistance for the injured as needed

Call 911 for emergency medical assistance.

You can also call TMU Security at 416-979-5040, 24 hours a day, 7 days a week.

If there is a critical injury, do not disturb the scene. Follow the steps above.

Step 3: Notify the placement coordinator

Promptly notify the TMU placement coordinator.

Step 4: Complete the Internal Near Miss, Incident and Injury Form within 24 hours

Complete the  (google form) Internal Near Miss, Incident and Injury Form (external link)  within 24 hours, or as soon as you are aware of the incident.

If the injury requires medical care or time off work, complete the WSIB Form 7 within 48 hours

If the injury requires medical care or time off work, you will be required to complete two forms with the TMU placement coordinator to the student trainee’s institution within 48 hours:

If you are a placement coordinator with a student trainee who has been in an incident or injury while on campus, follow these steps:  

Step 1: Notify the student trainee’s institution

After hearing about the incident or injury from either the student trainee or placement supervisor, notify the student trainee’s institution.

Step 2: Request the Letter of Authorization to Represent Placement Employer

Request the Letter of Authorization to Represent Placement Employer (a copy of which can be found in  (PDF file) Appendix C of the Guidelines for Workplace Insurance for Postsecondary Students of Publicly Assisted Institutions on Unpaid Work Placements) from the student trainee’s institution, and complete the “Placement Employer” section.

If the injury requires medical care or time off work, complete the WSIB Form 7 within 48 hours

If the injury requires medical care or time off work, within 48 hours, you will be required to complete two forms with the TMU placement supervisor to the student trainee’s institution:

If a non-employee has been hurt

If a non-employee (e.g. student, visitor, guest, volunteer, contractor) is involved in an incident or injury, they and the person in charge or the supervisor (e.g. professor, instructor, teaching assistant, chair, academic director, event organizer, project manager) must follow these steps:

If you are a non-employee who has been in an incident or injury while on campus, follow these steps:

Step 1: Seek first aid or medical emergency assistance as needed

Call 911 for emergency medical assistance.

Step 2: Notify those in charge immediately

If applicable, notify the supervisor or others in charge (e.g. professor, instructor, teaching assistant, chair, academic director, event organizer, project manager) immediately of any injury, even if it is an injury which does not require medical attention or lost time.

Step 3: Complete the internal Near Miss, Incident and Injury Form

Complete the  (google form) Internal Near Miss, Incident and Injury Form (external link)  and submit it to Toronto Metropolitan University Environmental Health and Safety within 24 hours or as soon as you are aware of the incident.

If you are a supervisor (e.g. professor, instructor, teaching assistant, chair, academic director, event organizer, project manager) or person in charge with a non-employee (e.g. student, visitor, guest, volunteer, contractor) who has been in an accident while on campus, follow these steps:  

Step 1: Go to the scene immediately

As the supervisor or person in charge, you are obligated to go to the scene as soon as you are notified to assist and control other potential incidents or injuries.  

Step 2: Call first aid or medical emergency assistance for the injured as needed

Call 911 for emergency medical assistance.

You can also call TMU Security at 416-979-5040, 24 hours a day, 7 days a week.

If there is a critical injury, do not disturb the scene. Follow the steps above.

Step 3: Investigate

Promptly investigate the incident to determine the cause. Cooperate but do not speculate or make assumptions on the cause of the incident. Do not sign any statements except as required by local law enforcement.

Step 4: Complete the Internal Near Miss, Incident and Injury Form within 24 hours

Complete  (google form) Internal Near Miss, Incident and Injury Form (external link)  within 24 hours, or as soon as you are aware of the incident.

The university’s risk and insurance officer is responsible for managing all losses and/or claims that may impact on the university’s insurance coverage. In the event that you are a non-employee (e.g. student, volunteer, guest, contractor) who has experienced any loss (e.g. personal injury, property damage, automobile), you or the supervisor/person in charge involved must:

  1. Inform the local police department or TMU Security (416-979-5040) so that there is an official report of the incident, accident or injury. They will also provide emergency medical assistance and coordinate with local authorities as necessary.
  2. Contact the risk and insurance officer at 416-979-5000, ext. 553772 or insurance@torontomu.ca.

The risk and insurance officer will assess the claim and will notify the appropriate insurance carrier as required.
