Incident Reporting

Have you been injured in a practice setting or involved in a clinical incident? Make sure to complete the  (google form) Clinical Incident Reporting Tool (external link) 

Workplace Safety and Insurance Board (WSIB) reporting is required if your injury or incident may result in or has resulted in any or all of the following:

  • Health care by a medical practitioner (i.e. physician, chiropractor, dentist, hospital and/or health facility)
  • Loss of time from placement and/or university beyond the date of the incident
  • Exposure to and/or contraction of a communicable disease (i.e. TB exposure, needlestick)

In the event of a practice injury or incident requiring first aid treatment only, you do not need to make a WSIB claim. Please follow placement agency/facility protocol for reporting and obtaining medical attention if required. If ongoing  medical attention is required, please seek follow-up from your family practitioner of the Toronto Metropolitan University Medical Centre. 

First aid is defined by the WSIB as a “one-time treatment or care and any follow-up visit(s) for observation purposes only. First aid includes, but is not limited to: cleaning minor cuts, scrapes, or scratches, treating a minor burn, applying bandages and/or dressings, applying a cold compress, cold pack, or ice bag, applying a splint, and changing a bandage or a dressing after a follow-up observation visit”.

If the practice incident is an accident or injury that necessitates WSIB reporting, please follow placement agency/facility protocol for reporting and obtaining medical attention if required. If ongoing medical attention is required, please seek follow-up from your family practitioner or the Toronto Metropolitan University Medical Centre.

Notify the clinical instructor or faculty advisor as soon as possible on the day of the incident.

Obtain, complete and submit the following WSIB documentation. (Please note your documentation may be required from your clinical instructor/preceptor)
